We could talk about this topic for a long time and I am sure there are differing opinions but I think it is vital that we recognise the barriers to breastfeeding in Ireland. I am a health professional and I most certainly feel the pressures that we are feeling in our health services. Its frustrating for us all that we can't give it our all to the needs of our patients at times! And I am sure this goes for breastfeeding supports in the maternity services and community. I think we have come a long way and we are slowly changing our perception on breastfeeding... its benefits.. to mothers babies and really the health service overall! My breastfeeding journey began over six years ago when I had my first little girl Lily-Losa. I was 28 living in New Zealand with my husband Rob. I assumed I knew a good bit about breastfeeding before I had Lily given all the experience I had working in a Paediatric Adult Maternity hospital. I met a lot of new mothers post-delivery in both settings. However like all first-time mothers your experience of birth and breastfeeding is really one that has a lot of challenges! Lily was a week overdue and when she decided to arrive, she came FAST. I had Lily in the maternity hospital where I had just arrived from the birthing centre due to meconium being in my waters when it broke. I had her very quickly and ended up with a quite a bad tear and a postpartum haemorrhage. I got to hold Lily and latch her briefly but was quickly whisked off to theatre. I was besotted and in that newborn bubble but also completely shook. I lost over 2 litres of blood and had been in surgery for a good few hours. The care in the maternity ward was wonderful like all health settings they are busy and overworked but looked after me with such kindness. As soon as I returned from theatre, I was latching and unlatching, trying to get the most comfortable hold while recovering from surgery and regaining the strength to move. It was overwhelming, and I felt helpless. By Day 3 I had received 2 blood transfusions and an iron infusion, my milk was coming in alongside my emotions! My breasts were engorged and while I continued to feed Lily she got more upset. I called for help and eventually a midwife manager came. The ward was super busy with 2 sets of twins and triplets (can you imagine). It turned out I was so engorged that there was barely any milk being transferred by Lily-Losa! I was so upset. I was taught to reverse pressure massage my breasts and told I would need to express milk to give extra to Lily. She had lost over 10% of her body weight at this stage. I was a mess and blubbered all the way to NICU to purchase a breastpump! The journey didn't end there I got help and was delighted to feed for 1 year!! #breastfeeding#breastfeedingsupport#ibclc#postpartum#newmother#breastfeedingireland

Engorgement is a real pain in the following days after delivery if not managed well you may be at risk of clogged ducts and mastitis. A few tips to help you relieve yourself: Feed directly from baby as frequently as possible. There is no schedule or routine but every 1.5 hours to 2 hours you should express yourself or feed baby. Hand express a little before you direct feed it can relieve the fullness. Apply cool flannels or ice packs in between feeds and as much as tolerated. Warmth can help with milk flow, so try have a hot shower or warm cloths around. Pop Gel compresses or breast pads in fridge and apply to areola when you can this will help with decreasing swelling in this area. Reverse pressure softening or massage starting at the nipple with gentle compression outwards to breast tissue and armpits will help with lymphatic drainage. Wear a supportive bra that is not too tight fitting. As always you are doing an amazing job and all these changes in your body and struggles that you face will be so worth it even if you breastfeed or not! Breastmilk is really the most wonderful source of nutrition for your baby. There are so many amazing changes in its makeup to cater for the needs to your baby as it grows. I also believe it is important to have the knowledge to help you make the best decisions for your little one. I am fully aware of the need and want to formula feed but I believe that knowing the makeup of both breastmilk and formula will help you on your journey whatever you may end up choosing to do. So..... Be careful with formula top ups in the beginning it can make baby feel more full, prolonging spaces between feeds and thus inhibiting the production of milk supply. Give small amounts if needed but preferably hand express or pump giving top ups of breast milk. If using formula become equipped with the knowledge of formula and what you are actually using don't be fooled by the marketing from infant formulas stating the need for use of follow on formula or toddler formula the ratios of casein increase in these and can be heavier on stomach. Lastly hungry baby formulas generally have a higher casein ratio this will kake your baby feel fuller but may cause constipation and stomach issues. Consult a medical professional, dietician or lactation consultant if you are concerned about intolerance or allergies in your child. Breast milk can and will change colour and there are a variety of reasons for this. The colour change is related to the changes in composition of milk from your own personal nutrition to the medication or supplements you may be taking, infections, ruptured blood vessels or illness. While mostly harmless some colours such a pink or red or black will need further investigation. Breastfeeding benefits are really endless! The research is still ongoing and unfolding so many positive outcomes for both mother and baby! As a Paediatric ICU nurse I see the need for both breastmilk and formula use in the medical environment! Any form of breastmilk is really a gift so all mothers should be proud of themselves for trying and giving their little ones some truly magical goodness! The benefits above are related to both long and short term breastfeeding where some of the benefits do increase the longer you breastfeed! ALCI is an All-Ireland Association promoting International Board Certified Lactation Consultants, known as 'IBCLC’s' in Ireland, as an unincorporated society. IBCLC meaning Internation Board Certified Consultants.It is ran voluntarily by a group of great adminstrators. It a great resource to help you find the support you need within the area you live. It provides information on both public and private help for lactation support. Only Board Certified Lactation Consultants are available on this website. This ensures you are going to get the most up to date and evidence based advice and support related to breastfeeding and human milk. It is also a great website to visit to look for resources related to breastfeeding. I have met so many new mothers who had never heard of this helpful domain so definitely take a look! True low milk supply can be caused by hypoplasia of the breast. This is due to an insufficent amount of glandular tissue being present. This is why a thorough assessment of mother and baby needs to be done by a Lactation Consultant. However there MAY be other potential causes of low milk supply ...that can be helped! Medical conditions from mother such as: •PCOS, fertility problems,Thyroid issues •Blood loss after delivery °Prolonged labour •Retained placenta •Separation from baby •Delay in expression/feeding •Poor latch •Maternal Age •Anxiety and Exhaustion Alot of these reasons can be quite common in our society today...what mother isn't exhausted or anxious..many babies do get separated for monitoring and prolonged labours I am sure all first time mothers can resonate with this. A recent study by Bhasin et al (2025) revealed that infants who are not exclusively fed colostrum in the first few days of life are at a higher risk of developing peanut allergies by 12 to 18 months of age. The research highlights the vital role colostrum plays in nourishing and shaping a baby's immune system, with data showing that 46% of infants who were only partially colostrum-fed had increased allergy risks. Fascinatingly, none of the infants who received at least nine colostrum feeds within the first 24 hours developed peanut allergies, underscoring the protective power of this natural superfood. Skin to Skin is so significant for both mother and baby. Some of you may have heard of the 'Golden Hour' where it is recommended for the baby to be placed on the mothers chest post birth for up to one hour... and why??? well there is so many reasons!! It calms and relaxes both mother and baby Regulates the baby’s heart rate and breathing, helping them to better adapt to life outside the womb It stimulates digestion and an interest in feeding Regulates temperature Enables colonisation of the baby’s skin with the mother’s friendly bacteria, providing protection against infection Stimulates the release of hormones to support breastfeeding and mothering. It is not only important in the first few hours of life but also for the rest of your breastfeeding journey.
